Description: Pathways Home Health, Hospice & Continuous Care is a state-licensed, Medicare-certified home health agency providing skilled, intermittent home health and hospice services at home. An individualized health care plan is developed for every patient by the multi-disciplinary home care team, with consideration to both health and personal needs. Services are provided under physician direction and are available seven days a week. Nurses are on-call 24 hours a day for consultation. The home care team consists of nurses, home health aids, medical social workers and physical, occupational and speech therapists. The patient, family and attending physician are considered members of the home care team. Specialized programs include home infusion/IV, cardiovascular, diabetic, pulmony and ostomy. Other programs are designed to help those with acute, chronic and terminal illnesses successfully manage care at home. Pathways Hospice supports the care decision of the terminally ill person and family. Attention is paid to coordination of medical and support services, pain control and physical comfort, emotional and spiritual support and enhancement of the quality of life. Individual and group bereavement services are also provided. The hospice team includes the patient's social workers, volunteers, a pastoral counselor and others as needed. Services are primarily provided in the home, but a program has been developed for nursing facility residents. Pathways Home Health, Hospice & Continuous Care is part of a family of home care services affiliated with El Camino Hospital and Sequoia Hospital. The goal is to provide comprehensive, high-quality services while making access to services easy for patients and families as well as physicians and others coordinating their care. The other members of the Pathways family include Pathways Hospice Foundation and Pathways Continuous Care- a state-licensed provider of provider of private duty nursing and attendant services.
